About Us
- BIENESTAR is a project focused on promoting the full and collaborative integration of behavioral health services and primary care services, including medical, dental, and other primary service team care from MHC, Inc. In addition, it is intended to maximize and contribute collaborative work between MHC, Inc. physical and behavioral health professionals who work as a team to ensure that patients are receiving all appropriate services necessary for overall health well-being.
- MISSION: Promote the integration of physical health and behavioral health services under a model focused on the holistic needs of the patient.
- VISION: We are committed to promoting education and integrated health care to promote the holistic well-being of our patients, creating a biopsychosocial-spiritual balance.
- REQUIREMENTS: (1) Over 18 years of age. (2)Diagnosis or evaluation of Behavioral Health. (3)Health conditions (Diabetes, Hypertension, Cancer, HIV, Hepatitis, and Arthritis, among others), (3) Want to make a change in your lifestyle to improve your physical and Emotional Health, (4) Optional, (5) Substance Use Diagnosis.
