About Us
- The Transition Clinic (CT) at MIGRANT is an innovative clinical program that provides a medical home for individuals with a chronic illness who have recently been released from prison. Community Health (TSC) workers, who also have a history of incarceration, guide our patients through transitioning out of prison and navigating the health care system.
- We believe that people with a history of incarceration: (1) Should be treated with respect, (2) Should have high-quality services specifically tailored to their needs, (3) Have the right to confidential services, (4) Have the ability to make decisions about your own health care.
- Support in search of: (1) Social reintegration, (2) Housing, (3) Employment and study, (4) Medical coverage, (5) Nutritional assistance program, (6) Transportation, (7) Financial assistance, (8) Tiered Discount Program and (9) Treatment and Detox Programs.
